The following forms will be temporarily unavailable due to system-related measures:

• Email Address Registration
• Reserve Material Request
• Request Form for Items in the Faculty Research Building Closed Stacks
• Donation Application Form
• Publications Order Form
• Purchase Request (for Faculty and Staff)
• Purchase Request (for Students)
• Reservation Form for the Video Conference Room This form is now available again.

Suspension begins
5:00 p.m. on Wednesday, October 21, 2026 (JST)

The resumption date has not yet been determined. Alternative ways to submit requests through each form during the suspension will be announced as soon as they are confirmed. We will also post an update here once the services have resumed.
