Temporary Suspension of Access to Some Application Forms (SFC)

Due to system maintenance, access to the following forms for Shonan Fujisawa Media Center and the Nursing and Medical Care Library will be temporarily unavailable from off campus.

・Designated Materials Request Form
・Purchase Request Form
・Email Address Registration/Change Form

Wednesday, October 21, 2026, from 5:00 p.m.

The resumption date has not yet been determined. Information on alternative procedures for each form during the suspension period will be provided as soon as it becomes available. We will also notify you once access has been restored.
